Taping ® Therapy Method and Sensory Integration for the Treatment of Gait in Children with Autistic Spectrum Disorder: Case Report

Abstract

The Autistic Spectrum Disorder is characterized by communicative, interactional, behavioral and functional kinetic difficulties which interfere  in sensory-motor skills. This study  is a case report whose objective is to present the results of the intervention of Occupational Therapy with a 4-year-old child, with Autistic Spectrum Disorder, emphasizing the gait characterized by support in the tiptoes.  The sensory modulation was evaluated by the sensory profile, without indicative of tactile hyper-reactivity and the sensory perception assessed by Sensory Integration Praxis Tests, which showed low perception of vestibular, tactile and proprioceptive systems. It is suggested as a hypothesis that the improper gait was related to lowered sensory perception  and not to the tactile hyper-reactivity. Thus, sensory Integration therapy associated with the Therapy Taping ® method for relaxation of the gastrocnemius and plantar tactile stimulation with a frequency of twice a week was begun. Initially,   the control of frequency of verbal command for plantar support and the identification of the context of inadequate gear within the period of four hours during a week was begun.  The result showed that fifteen verbal commands  were performed, and  the contexts were variable. After twelve weeks, this procedure was repeated for the count of frequency of verbal commands and it was identified that there was a reduction from fifteen to five verbal orientations and the context was restricted to situations of euphoria. This study has demonstrated that the Therapy Taping^® method  associated with sensory integration to promote the appropriate pattern of gait in children with autistic spectrum disorder that presents no tactile hyper-reactivity can be efficient.

O Transtorno do Espectro Autista carcteriza-se por dificuldades comunicativas, interacionais, comportamentais e cinéticas funcionais que interfererem nas habilidades sensório motoras. Este estudo, trata-se de relato de caso cujo objetivo é apresentar os resultados da intervenção de Terapia Ocupacional com uma criança de quatro anos de idade, com Transtorno do Espectro Autista, enfatizando a marcha caracterizada pelo apoio na ponta dos pés.  A modulação sensorial foi avaliada pelo Perfil Sensorial, sem indicativos de hiper-reatividade tátil e a percepção sensorial avaliada pelo Sensory Integration Praxis Tests, que demonstrou baixa percepção dos sistemas vestibular, tátil e proprioceptivo. Sugere- se como hipótese que a marcha inadequada estava relacionada à percepção sensorial rebaixada e não à hiper-reatividade tátil. Sendo assim, deu-se início à terapia de Integração Sensorial associada ao método Therapy Taping^® para relaxamento de gastrocnêmio e estimulação tátil plantar com frequência de duas vezes por semana. Inicialmente foi realizado o controle de frequência de comando verbal para apoio plantar e a identificação do contexto da marcha inadequada no período de quatro horas durante uma semana.  O resultado demonstrou que eram realizados quinze comandos verbais e os contextos eram variáveis. Após doze semanas, repetiu-se o procedimento para a contagem de frequência dos comandos verbais e identificou-se que houve redução de quinze para cinco orientações verbais e o contexto estava restrito a situações de euforia. Este estudo demonstrou que o método Therapy Taping^® associado à Integração Sensorial para favorecer o padrão adequado de marcha em crianças com Transtorno do Espectro Autista que não apresente hiper-reatividade tátil pode ser eficiente.
